Client registration form
This form must be completed and submitted prior to receiving veterinary care.

All client information is kept confidential under CVO by-law, and it allows us to ensure your contact details are accurate along with having a back-up contact in case of an emergency with your animal. It also contains information on our hospital policies.

Please note that to remain an active client and receive priority of care during office and after-hours, we must be your primary veterinarian and have a current veterinary-client-patient relationship. This means we need to see your cat for wellness visits every 24 months at minimum, though annual exams are recommended for healthy patients under the age of 15, and semi-annual visits for geriatric patients or those with on-going health conditions.

*We do not offer after-hours emergency care*. Your animal's condition will be triaged through Smart Vet and directed to the nearest 24 hour animal hospital (Barrie, Toronto, Guelph) for after-hours care or when a veterinarian is not in the office.
